Here’s a checklist of things you’ll need for your child’s first appointment:
- Arrival Time: Please arrive 10 minutes early for your child’s initial visit to allow time for paperwork completion.
- Questions: Prepare a list of questions or concerns you’d like to discuss during your first meeting with our pediatric occupational or speech therapist.
- Documents: Don’t forget to bring essential documents, including your photo ID, a copy of your insurance card, and completed schedule request form.
- Insurance Verification: Prior to your appointment, contact our office at 586-293-1234 to confirm your insurance benefits.
- Medical History: Be ready to provide information about your child’s medical history, past treatments, current medications, and any other relevant details.
- Symptoms and Limitations: Create a list of your child’s symptoms, limitations, sensory or emotional difficulties. How long has your child been experiencing this issue, and have they encountered similar problems before?
The more information you can provide about your child’s condition, the better our pediatric occupational or speech therapists can assess and address their needs.
What to Expect at Flourishing Lives During Your Child’s First Appointment:
- Check-In: Upon arrival at our pediatric therapy clinic, you’ll need to check in at the front desk. We’ll collect insurance information and any paperwork requested by our administrative team.
- Initial Evaluation: Your child’s first appointment will include an initial evaluation by one of occupational or speech therapists. During this evaluation, our therapist will review your child’s medical history and assess your child’s current condition, pain, illness, or injury. Honest and open communication is crucial to understanding your child’s needs fully.
- Physical Examination: Our pediatric therapist will perform a thorough physical examination to evaluate your child’s gross motor, fine motor and visual motor skills, balance, movement patterns, sensory and emotional regulation. Based on these findings, they will develop a personalized therapy plan for your child.
- Discussion: The therapist will explain their findings and provide information about the evaluation. Each subsequent session will build upon the initial evaluation for the most significant impact.
- Goal Setting: We’ll discuss your child’s goals and determine the recommended frequency and duration of their pediatric therapy sessions based on clinical expertise and reasoning.
- Documentation: Our pediatric therapists will document their findings and treatment plan, which can be shared with other medical professionals of your choice. The primary aim of our treatments is to expedite your child’s progress and achieve lasting, meaningful results.
- Scheduling: Before leaving your child’s initial appointment at Flourishing Lives, you’ll provide them with a completed schedule request form so they can schedule your child’s entire plan of care.
